About the Role
As a Service Coordinator, you will deliver high-quality, person-centred tenancy support to older people across Swansea. Working within a Psychologically Informed Environment (PIE) framework, you will support individuals who may be facing complex challenges, helping them maintain their homes, strengthen resilience, and avoid homelessness.
The Service Coordinator will build positive relationships with service users, develop tailored support plans, work alongside partner agencies, and ensure services are delivered to the highest standards. This rewarding role offers the opportunity to make a genuine difference every day while supporting people to live independently and achieve their goals.
About You
We're looking for a Service Coordinator who is compassionate, proactive, and solutions focused. You should be able to communicate effectively, manage competing priorities, maintain accurate records, and work collaboratively with colleagues and stakeholders.
Essential:
We welcome previous experience working as a Support Worker, Housing Support Worker, Tenancy Support Worker, Community Support Worker, Care Coordinator, Homelessness Prevention Officer, Case Worker, Social Care Worker, or in other customer-focused support roles.
